Always consult your Doctor or other healthcare provider before taking advice, changing your diet or starting an exercise program. If at any stage during your membership of Lindsay Taylor (Taylored Pilates) your medical status changes you should consult your doctor or other suitable health professional at the earliest opportunity prior to continuing with your exercise program. It is your responsibility to make your Lindsay Taylor (Taylored Pilates) instructor aware of any changes in your health condition whilst you are a member.
I recognise that Lindsay Taylor (Taylored Pilates) is not able to provide me with medical advice with regard to my medical fitness and that the information is used as a guideline to the limitations of my ability to exercise safely. I also state that I wish to participate in activities which may include aerobic exercise, resistance training and stretching.
I realise that my participation in these activities involves the risk of injury and even the possibility of death. Furthermore, I hereby confirm that I am voluntarily engaging in an acceptable level of exercise, which has been recommended to me.
I assume full responsibility for any injury, loss or damages that may result while attending, exercising or participating in any activity hosted or provided by Lindsay Taylor (Taylored Pilates).
I hereby waive all claims against Lindsay Taylor (Taylored Pilates) Including their instructors, or partners of individually or otherwise for any and all injuries, claims and damages that I may incur on or near the premises, or during and online or outdoor session.
